                                  MEDIATION: Some Helpful Forms

          The following forms may be helpful in preparing for mediation and addressing the kinds of issues covered in settlement agreements concerning child custody and visitation, child support, alimony, monetary award and property division (real property, personal property, pensions, etc.) and other family law matters.

Financial Statement (Short Form): used in child support cases where alimony is not at issue and the parties' combined annual income is $120,000 or less.

Financial Statement (Long Form): used in all alimony cases, and also in child support cases where the parties' combined annual income exceeds $120,000.

Statement of Marital Property and Nonmarital Property: used in cases involving property division, pensions, etc.

A Model for a Parenting Plan, by the American Academy of Matrimonial Lawyers (AAML): provides a useful checklist for matters to be considered in child custody and visitation cases.

The Montgomery County Guidelines for Effective Parenting, that covers parenting plan considerations and may be a helpful checklist along with the AAML Model for a Parenting Plan.

The Mediation Issues Checklist, that suggests many issues to be addressed in cases involving child custody, visitation, child support, alimony, monetary award and property division (real property, personal property, pensions, etc.), and other marital settlement or property settlement agreements.
